TELEPHONE (ANI) REGISTRATION / APPLICATION FORM

Terms & Conditions:
1. You must complete this form for each T-Lock unit ordered. If you purchsed multiple units, please submit one form for each telephone number.
2. All business applications are subjected to approval by our public telephone provider based on the expected usage of the public telephone service.
3. HiQ's Public Telephone service provider reserves the right to decline your application based on our own assessment on the expected usage of the publicc telephone service.
4. Acceptable business applications include vacation rentals, lodging rooms, lobby rooms, courtesy phones, or any other business application that is expected to make frequent use of HiQ's provided long distance service.
5. Non-acceptable business applications include protection of fax lines or any phone lines to mainly prevent dialing of unathorized calls. Please refer to our T-Lock with Optional call Timer.
6. Declined applications will be required to pay $27 per unit prior to shipping.
